There are a range of note taking apps which allow a PDF template to be used underneath the hand written notes. This enables a paperless workflow to be created meaning that you will capture and store your interview notes without paper. Why not paste the CV or resume of the interviewee into the document as well?
The PDF includes a continuation sheet which can be replicated as many times as needed in the app.
